to all the teachers, better late than never i guess, plz ask students to return dental forms signed by the parent or guardian if they do not want the service. many parents told me last year that they would have loved to participate had they been given the form. students are making choices for their parents. if they have lost the forms, i have extra or they can bring a note from parent or guardian stating they are not interested or have a dentist.
